Smile Index Worksheet

The Smile Index exists so that the patient, the dentist, and the lab share the same vocabulary about what is being changed. Score the current smile, score the target, and the gap between the two is the plan.

How to use it
- 01Patient is seated upright. Photographs on the consultation monitor.
- 02Score the current smile on each axis. Write the number.
- 03Score the target smile on each axis, with the patient. Write the number.
- 04Calculate the gap.
- 05Attach the sheet to the lab brief.

The five axes
- Length
- Incisal-edge position relative to the lower lip line at rest and at a relaxed smile. Score 1 (much shorter) to 5 (much longer than ideal).
- Width
- Proportion across the smile zone — centrals, laterals, canines. Score 1 (narrow) to 5 (wide).
- Axis
- Long-axis inclination of centrals and laterals. Score 1 (heavily inclined) to 5 (heavily upright).
- Midline
- Coincidence with facial midline, cant, deviation. Score 1 (large deviation) to 5 (perfect alignment).
- Harmony
- Gingival architecture and zenith line across the smile zone. Score 1 (asymmetric) to 5 (harmonious).

Working with the lab
Numbers on the sheet, not adjectives. The lab adjusts on specifics, not feelings. If the patient described something in their own words during scoring, capture the words verbatim under the matching axis. The lab will read both — and act on the number.
