Surgical placement of dental implants

Implant Consent Form

This form covers the surgical placement of one or more dental implants and the prosthetic restoration that follows. The procedure is staged across several months. The form sets out the clinical sequence and what is asked of you between visits.

Stages of treatment

  • ·Diagnostic imaging — cone-beam CT scan and full intra-oral records.
  • ·Surgical placement — implant fixture installed under local anaesthesia.
  • ·Healing — three to six months for osseointegration, depending on site.
  • ·Prosthetic — final crown or bridge cemented onto the implant.

Risks you should understand

  • ·Surgical risks of bruising, swelling, and minor bleeding in the first 48 hours.
  • ·Risk of implant failure to integrate — uncommon but possible. The studio's protocol for this is replacement at no additional surgical cost within the first year.
  • ·Nerve sensitivity in lower-jaw cases — explained in detail before consent.
  • ·Need for bone graft or sinus lift in some cases — disclosed in the diagnostic stage.

What we ask of you

  • ·Avoid smoking for the first two weeks after surgery — strongly preferred for the lifetime of the implant.
  • ·Use the prescribed mouthwash and follow the soft-food protocol for the first week.
  • ·Attend every follow-up — the schedule is non-optional.
  • ·Maintain professional hygiene visits every six months, for life.
