Veneer Treatment Consent Form

This form is the record of our conversation. It exists so that you, the patient, and we, the studio, share the same understanding of what is about to happen, what is expected, and what is not promised.

What we are agreeing to do

Place ceramic veneers on the teeth listed on the treatment plan attached to this form. The veneers are made in our own lab from a proprietary porcelain layered above pressed e-max. The procedure is minimal-prep unless the clinical plan specifies otherwise.

What you can expect

  • ·Two or three visits with a mock-up between. About two weeks from start to finish for most patients. Sometimes longer when travel asks for it — never rushed.
  • ·Photographic documentation of every visit — kept private to the studio unless a separate marketing-photo consent is signed.
  • ·Temporary veneers between preparation and final cementation.
  • ·Sensitivity for a typical window of 48 hours to ten days after final cementation.
  • ·A night-guard fitting two weeks after final cementation. The night-guard is part of the treatment, not optional.

What we do not promise

We do not promise a specific shade beyond BL2 with controlled translucency. We do not promise zero sensitivity. We do not promise that veneers placed today will never need attention. We do promise to do the work to the same standard as the photographs you have seen, and to be available to you for the lifetime of the result.

Risks you should understand

  • ·Sensitivity during the first ten days, occasionally longer.
  • ·Possibility of a veneer becoming loose or chipping during normal life.
  • ·Minor recession of the gumline over years, exposing a margin.
  • ·Need for night-guard use to protect the work from bruxism.
  • ·Need for replacement, on average, after fifteen to twenty years.

What we ask of you

  • ·Wear the night-guard.
  • ·Use the prescribed toothpaste in the first month.
  • ·Do not whiten with peroxide toothpaste over the veneers — it will not change their shade.
  • ·Contact the studio directly if anything escalates beyond normal sensitivity.
  • ·Attend the two-week and three-month follow-up.
